MEGA驗廠從驗廠內(nèi)容上主要可分為“社會責(zé)任、反恐、質(zhì)量”等，通常社會責(zé)任審核，為突襲驗廠，待通知期為三周，該時間段內(nèi)任何一天審核員都有可能會來到工廠。OMEGA驗廠過程中主要關(guān)注車間現(xiàn)場消防安全、職業(yè)健康、文件資料方面，會集中關(guān)注不一致的問題。OMEGA驗廠審核員有可能在車間找資料。

OMEGA社會責(zé)任驗廠審核文件清單

1、Business License/ Certificate of Registration 營業(yè)執(zhí)照副本；

2、Factory map/ floor plan (If any) 工廠平面圖（如有）；

3、Payroll records of past 12 months with employees’ signature 過去 12 個月的工資表(含員工簽名)；

4、Piece rate records of past 12 months (If applicable)計件工人過去 12 個月的計件記錄（如適用）；

5、Attendance records of past 12 months 過去 12 個月的考勤記錄；

6、Official overtime waiver/ Comprehensive Working Hour System permit (If applicable)當(dāng)?shù)貏趧泳峙l(fā)的加班批文或綜合工時批文（如適用）；

7、Production records of past 12 months and the other related records（i.e. Daily production records, daily QC report, warehouse material in/out records, etc.)過去 12 個月生產(chǎn)運作記錄及其它生產(chǎn)相關(guān)記錄 (如：生產(chǎn)日報表，品管日報表，倉庫收發(fā)料記錄等)；

8、Social insurance documents (payment receipt, registration certificate, list of participating employees, waiver approved by Social Insurance Department etc.) 社保文件(已購買社會保險的收據(jù)，社保登記證,社保參保人員名單，社保局出具的社保批文等）；

9、Leave policy and records, including maternity leave, annual leave, work injury leave, sick leave,marriage leave and funeral leave etc.假期政策及記錄，包括產(chǎn)假，年假，工傷假, 病假，婚假和喪假等；

10、Employee casual leave records 員工事假記錄；

11、Employee resignation/ termination record 員工辭職/辭退記錄；

14、Juvenile Workers (age 16-18) registration, health check report and job assignment records (If applicable)未成年工(16-18 周歲)的勞動局備案登記, 體檢報告及崗位安排記錄 (如適用)；

15、Employee handbook and factory rules 員工手冊及廠紀(jì)廠規(guī)；

16、Health & Safety Representative training record, certificate or equivalent 健康安全負(fù)責(zé)人培訓(xùn)記錄、證書或類似證明文件；

17、Employees orientation and health & safety training records, includes but not limited to fire safety,chemical safety, machine safety, electric safety, PPE, occupational disease, etc. 員工入職培訓(xùn)及職業(yè)健康安全培訓(xùn)記錄，如消防安全、化學(xué)品安全、機(jī)器設(shè)備安全、電氣安全、個人防護(hù)用品、職業(yè)病防治等；

20、Fire inspection certificate and report issued by Fire Department or third party consultant firm (If any)

21、Emergency evacuation procedure and fire drill report with photos 緊急疏散程序，消防演習(xí)報告及照片；

23、Certificate of trained first aid personnel 急救人員培訓(xùn)證書；

24、Certificate and license of in-house doctors and nurses (If applicable)駐廠醫(yī)護(hù)人員的資質(zhì)證書/執(zhí)照（如適用）；

25、Chemical list and inventory records, Material Safety Data Sheet (MSDS) 化學(xué)品清單及倉存記錄, 化學(xué)品安全技術(shù)說明書；

26、Hazardous elements (i.e. air quality, noise, etc.) evaluation report for workplace 工作場所職業(yè)危害因素(如空氣質(zhì)量, 噪聲等)評價報告；

27、Occupational health examination report of employees (If applicable)危險崗位工人的職業(yè)病體檢記錄（如適用）；

28、Operating instruction for heavy machinery operator (If applicable)重型機(jī)器的操作指導(dǎo)書（如適用）；

29、Blade policy and record (If applicable)利器管制程序及記錄（如適用）；

30、Special equipment registration, regular inspection reports and operators’ licenses (e.g. boiler,
pressure vessels, elevator, lifting machine, in-house motor vehicle, etc.) (If applicable) 特種設(shè)備的登記證, 檢測報告及其操作人員的操作證（如：鍋爐，壓力容器，電梯，起重機(jī)械，廠內(nèi)機(jī)動車輛等）（如適用）；

38、Business license of labor dispatch company, contract signed with labor dispatch company, dispatched workers' personal files, ID copies, payroll and attendance records, labor contracts, social insurance payment evidence, etc. (if applicable) 勞務(wù)派遣單位的營業(yè)執(zhí)照，與勞務(wù)派遣單位簽訂的合同，勞務(wù)派遣工人的人事檔案，身份證復(fù)印件,工資,考勤記錄,勞動合同,社保繳費憑證等（如適用）；

OMEGA驗廠
